Preparing for your
initial behaviour assessment or home visit
Step 1. Veterinary Referral
Please note that the vet referral is a mandatory step for help with behaviour problems. If I haven't received the referral from your vet within 3 days of your booking, I will have to cancel your reserved slot, and we will have to re-arrange.
Therefore, please send the following email to your vet to ask for a referral as soon as you have booked your slot (and give them a quick ring after sending it to give them verbal consent):

I have booked a behaviour consultation/initial behaviour assessment for my dog with Zoe Price (CCAB) from Dog Behaviour Help.
As part of her professional consultation process, she requires a veterinary referral.
Could you please complete the referral form at the link below and share my dog's medical history?

The link includes both a digital submission and a printable template if that is more convenient for your practice. Please let me know if you need any further information.

Or if you prefer, you can also print the vet referral, and your vet can email it together with your dog's veterinary history to zoe@dogbehaviourhelp.co.uk
Optional: Additional information
You can send videos of your dog having a great time (e.g. to help me get a better overview of what your dog loves, and how your dog interacts with you or its environment).
You can also send me videos of the problem behaviour if you already have them. Please don't set a video up if it would distress your dog or make the situation potentially unsafe.
Where relevant, you can also send me the layout or pictures of your house. This is particularly useful for separation-related cases, any aggressive behaviour towards humans or households with multiple dogs.
